Health-Related Social Needs (HRSN) Housing Supports
Open Door HousingWorks provides Health-Related Social Needs (HRSN) Housing-Related Supports for eligible Oregon Health Plan (OHP) members through contracted Coordinated Care Organizations (CCOs). These Medicaid-funded, non-medical services are designed to stabilize housing during critical life transitions and reduce preventable health crises associated with homelessness.
HRSN housing supports may include outreach and engagement, housing navigation, tenancy-sustaining services, short-term rent and utility assistance, and other one-time housing stabilization costs, as authorized.
Services are eligibility-based, time-limited, and coordinated in partnership with health plans to support long-term housing stability and improved health outcomes.
